Become a FHASE Voting Member
L’Hôpital de la Communauté Haïtienne (Haitian Community Hospital) is operated under La Fondation Haïtienne de la Santé et de l’Education (the Haitian Health and Education Foundation), a non-government and non-profit organization.
As a community organization, we invite you to become a voting member of La Fondation Haïtienne de la Santé et de l’Education (FHASE). FHASE voting membership allows you to attend all Foundation Membership meetings and gives you voting rights in the Board of Directors election.
To become a voting member of FHASE, please fill out the following application and submit the annual fee of $100 (US).
